			<content:encoded><![CDATA[<p>Although it is rare for breast cancer to strike younger women, the fact remains that all women are at risk. And for those of childbearing age, the first sign and symptoms of breast cancer leading to a diagnosis can not only be upsetting and unexpected, but complicated as well.</p>
<p>Developing breast cancer at a younger age—in a woman’s 40s, 30s, even 20s—will mean making important and difficult decisions about one’s life and future perhaps much sooner than originally expected.</p>
<p>One concern is developing breast cancer during pregnancy, which although rare, can still occur. In this case, the treatment chosen will not only affect the patient and her body, but the growing baby inside her as well. It will depend on what stage of pregnancy she is in (first, second or third trimester) and what stage her cancer is in—such as whether or not it’s advanced.</p>
<p>Most pregnant women can have treatment for their breast cancer without affecting the baby. But some might be advised by their obstetrician or health-care practitioner—or even decide themselves—to terminate the pregnancy, more so if the pregnancy is in its earlier stages, in order to receive certain treatments that would be too risky otherwise. But it is essential to remember that it is a woman’s own decision—it is not medically necessary to terminate a pregnancy if the expectant mother is diagnosed with breast cancer. All it does is limit treatment options. Breast cancer itself will not affect the fetus—only certain tests and treatments will.</p>
<p>Generally speaking, tamoxifen, chemotherapy, radiation, and other drug-related therapies are avoided if the woman is pregnant because of their associated risks with birth defects. Tamoxifen, especially, is considered very unsafe because it is a hormonal therapy and is never recommended if the woman is pregnant or planning on conceiving.</p>
<p>Surgery—either a lumpectomy or mastectomy—is the most common and preferred method of treatment for breast cancer in pregnant women.</p>
<p>Another concern is whether or not breast cancer survivors can or should go on to have children after treatment and recovery. It’s a very controversial issue with firm advocates on both sides of the debate.</p>
<p>There are two main questions here, for both the medical and health community and breast cancer survivors wanting their own children: 1) Do certain breast cancer treatments affect fertility?; and 2) Is it actually considered safe to conceive and carry a baby to term following breast cancer and breast cancer treatments?</p>
<p>As far as fertility goes, there is no definite answer here. For chemotherapy, it depends on the age and what specific drug was used—some affect fertility more than others. And taking tamoxifen after chemotherapy to prevent recurrence is not recommended if the woman desires to become pregnant right away. Although tamoxifen is sometimes used as a fertility treatment, there is evidence to suggest that it damages developing embryos, and therefore is not considered safe to use.</p>
<p>Many doctors caution these women to wait several years to ensure receiving the best breast cancer treatment possible and to go past the point of the biggest threat of breast cancer recurrence. But some women decide to go ahead and have babies anyway, since it’s so important to them.</p>

Depression do not care what your age is, what gender you are, or even what your race or social class is. Depression can often make a person feel sad, helpless, hopeless, and irritable.  It is normal for people to have these feeling sometimes, but some people cannot just snap out of it and this is the difference between what is normal and major depression.  It is the determination and brutality of the emotions that determine the mental illness of depression from normal mood changes.</p>
<p>We are going to talk about different types of depression and how it can affect you. Depression is an illness that affects your body, mind, disposition, thought, sleep, energy, concentration, weight, and much more.  Depression is not a mood, it is not a sign of personal weakness, and it cannot be simply willed away.</p>
<p>Clinical depression is constant and can interfere significantly with an individual&#8217;s ability to function by emotional experiences of sadness, loss, or passing mood states. Clinical depression can be devastating to all areas of a person&#8217;s everyday life, including family relationships, friendships, and the ability to work or go to school.</p>
<p>Several things can cause depression. One of the causes can be attributed to a deficiency of certain neurotransmitters in the brain that signal from one nerve cell to another.  The immune system of a depressed person is usually very low and therefore ineffectively responding to diseases, including cancer.  The everyday stressors faced by people have simply become too much for many to bear, and reasonably so. In recent years, taking an anti-anxiety medication or anti-depressant has become as common as taking a multi-vitamin.</p>
<p><strong>Fight back<br />
</strong>here are a few tips to help you feel better when you are feeling down in the dumps. They are easy to do, easy to practice every day and they work!</p>
<p>1. Stand up straight, sit up straight. When your body is in alignment your energy can flow and when your energy is flowing freely, you can flow.</p>
<p>2. Smile! Yes, just smile. Easy to do and effective.</p>
<p>3. Repeat positive affirmations. Things like &#8220;I feel good&#8221;, &#8220;Positive energy flows through my body&#8221;, &#8220;I see the good in all&#8221;.</p>
<p>4. Listen to some music that you like. It doesn&#8217;t have to be anything specific, just something you enjoy. Certain types of music work better than others, but experiment and see what works for you. Studies have shown that Classical music and new age music work best.</p>
<p>5. Take some time out for yourself, relax and read a book, do something for yourself.</p>
<p>6. Meditate. Meditation is an excellent habit to develop. It will serve you in all that you do. If you are one who has a hard time sitting still, then try some special meditation CDs that coax your brain into the meditative state.</p>
<p>Try and practice them everyday, make them a habit. You will be surprised at how these simple exercises will keep the rainy days away. On a final note, if you are in a deep depression that you can&#8217;t seem to shake, please go see a doctor. This is your life and don&#8217;t take any chances.</p>

<p>Depending on what you do on a daily basis, you may need a chair with a pressure-relieving cushion, brake extensions, or other special options in order to make it effective. Since a wheelchair is designed to either fully provide or significantly add to your mobility, it’s vital that you get the right chair for you.</p>
<p>Unlike prescriptions for medications, which are very specific, a doctor’s prescription for a wheelchair often reads “a wheelchair”. This is partly because most General Practitioners are not experts in physical medicine and so they are not really qualified to make a suggestion as to exactly what type of chair you’ll require.</p>
<p>When possible, it is a very good idea to go through the selection process with a qualified physiotherapist or occupational therapist who knows what your daily routine and lifestyle look like so that they can help you get the chair that properly suits your needs.</p>
<p>Depending on what your requirements are, you may need to have your doctor write a somewhat specific prescription for insurance purposes. If you’ll be spending a significant amount of time in your chair, or if you have particular needs in terms of posture or support, these options can represent a considerable expense above a basic wheelchair system – for many insurers to provide payment, they require that these options be outlined by a physician, which means that you may need to have the prescription re-written by your doctor after an evaluation with a rehabilitation professional.</p>
<p>Although evaluations are not yet required by all insurers, most rehabilitation personnel consider them an absolute requirement – people always test-drive a car before buying it, and in the same way you should get to test-drive your wheelchair and have a professional opinion to ensure that it will do what you need it to.</p>
<p>Getting a professional evaluation before purchasing your wheelchair is the best way to ensure that you get a chair that will support you in doing what you need to do on a daily basis so that you can live comfortably and do the things that you want and need to do, so even if your insurer doesn’t demand one – insist on it!</p>

Gejala anemia yang kerap dirasakan adalah lesu, mudah letih, kulit pucat, pusing bahkan sakit kepala. Anemia banyak macamnya, namun yang tersering menyerang adalah anemia defisiensi besi (Fe) dengan wanita sebagai sasaran utamanya. Anemia defisiensi Fe bisa terjadi ketika kebutuhan tubuh akan zat besi meningkat, kurangnya asupan zat besi dari makanan atau adanya sumber perdarahan, misalnya saat haid pada wanita.</p>
<p>Karena itu, langkah pencegahan anemia yang sebaiknya diupayakan adalah dengan mencukupi asupan zat besi. Sedapat mungkin cukupilah kebutuhan zat besi dari sumber alami. Beberapa makanan seperti hati, jantung, kuning telur, kerang, ragi, kacang-kacangan dan buah-buahan kering tertentu mengandung zat besi dalam jumlah yang cukup tinggi. Zat besi dalam jumlah sedang, bisa diperoleh dari daging, ikan, unggas, sayuran berwarna hijau dan biji-bijian.</p>
<p>Sumber zat besi dari hewani akan diserap lebih baik oleh tubuh daripada yang berasal dari nabati. Penyerapan zat besi dapat dibantu dengan mengkonsumsi vitamin C, misalnya meminum jus jeruk, jus tomat, jus strawberi atau buah-buahan dan sayuran lainnya. Sebaliknya tanin yang terdapat dalam teh dan kopi dapat menghambat penyerapannya.</p>
<p>Salah satu trend wanita saat ini adalah mengkonsumsi suplemen kalsium atau susu tinggi kalsium maupun berbagai makanan yang ditambahkan kalsium. Sayangnya, penyerapan zat besi akan terganggu jika dikonsumsi bersamaan dengan kalsium. Karena itu, disarankan kedua suplemen tersebut dikonsumsi dengan jarak waktu sekira 1,5 – 2 jam. Hindari pula meminum suplemen zat besi dengan sumber kalsium, misalnya yoghurt dan susu.</p>
<p>Jika anda meminum multivitamin, perlu dicermati apakah didalamnya terkandung zat besi dan kalsium. Mungkin kita tidak akan mendapatkan zat besi dalam jumlah yang diharapkan dengan mengkonsumsi multivitamin tersebut. Agar lebih aman dan tidak membingungkan, sebelum mengkonsumsi suplemen yang mengandung zat besi ataupun kalsium sebaiknya berkonsultasilah terlebih dahulu dengan dokter. (Ginna M, dr.)</p>

<p>Tidak semua pasien yang berobat ke dokter harus menjalani berbagai macam pemeriksaan penunjang. Pemeriksaan darah di laboratorium, foto rontgen, CT scan, MRI dan lain sebagainya hanya akan dilakukan jika memang benar-benar diperlukan. Tetapi ada satu hal yang dapat dipastikan, setiap pasien pasti akan menjalani proses wawancara (tanya-jawab) dengan dokter seputar keluhan sakit yang dialaminya.</p>
<p>Wawancara tersebut dikenal dengan istilah anamnesis. Anamnesis yang baik akan mengalir lancar dan spontan layaknya sebuah obrolan. Suasana anamnesis yang nyaman seperti itu hanya bisa dicapai jika dokter maupun pasien bersikap ramah dan santun  serta memiliki kemampuan berkomunikasi yang baik. Mungkin terdengar sangat sepele, namun perlu diketahui bahwa sebuah anamnesis yang baik adalah modal dasar bagi seorang dokter untuk menegakkan diagnosis suatu penyakit.</p>
<p>Idealnya pasien dapat menggambarkan dengan jelas keluhan sakit yang dideritanya. Misalnya pasien dengan keluhan nyeri dada, akan sangat membantu jika ia dapat menyebutkan dan menunjukkan di bagian dada mana nyeri tersebut muncul. Bagaimana sifat nyeri tersebut, apakah seperti ditimpa benda berat, seperti ditusuk atau diremas, perih dan sebagainya. Sejak kapan nyeri itu mulai dirasakan dan apakah terjadi terus-menerus atau hilang timbul? Apa saja yang dapat memicu munculnya nyeri tersebut, aktivitas fisik, gangguan emosional atau kondisi-kondisi khusus lainnya yang dapat dijelaskan dengan bahasa pasien sendiri. Pasien pun diharapkan dapat menginformasikan hal-hal tertentu yang memperberat atau meringankan keluhan nyeri tersebut, juga keluhan-keluhan penyerta lainnya yang dirasakan berhubungan.</p>
<p>Cukup sampai disitu? Tentu saja belum, dokter juga akan menggali informasi seputar riwayat perjalanan penyakit tersebut, riwayat pengobatan, riwayat penyakit lain yang pernah diderita sebelumnya dan riwayat sakit dalam keluarga. Dalam hal ini keluarga pasien memegang peranan penting. Karena itu, disarankan anggota keluarga yang mengantar pasien adalah orang terdekat yang betul-betul memahami perjalanan sakit sang pasien. Sangat sering dijumpai seorang istri jauh lebih mengetahui riwayat perjalanan penyakit sang suami atau mungkin juga nenek yang lebih mengerti tentang sakit yang diderita cucunya. Untuk riwayat pengobatan sebelumnya, penting diingat kapan pengobatan itu dilakukan, obat apa saja yang diberikan dan bagaimana aturan pakainya. Jika ada obat-obat tradisional yang diminum sampaikan pula hal tersebut kepada dokter.</p>
<p>Uraian tersebut hanyalah sebuah contoh, tentu banyak pertanyaan lain yang akan diajukan dokter tergantung pada informasi yang dibutuhkannya. Penting untuk digarisbawahi, dalam proses anamnesis pasien/keluarga pasien jangan merasa seperti sedang dihakimi atau disalahkan. Perasaan seperti itu sangat merugikan karena akan banyak informasi yang bias atau bahkan disembunyikan pasien/keluarga pasien dari dokternya.</p>
<p>Pasien pun memiliki hak yang sama dengan dokter, pasien berhak untuk memperoleh berbagai informasi tentang penyakitnya. Tak perlu sungkan untuk bertanya pada dokter. Meskipun cara penyampaiannya berbeda-beda, percayalah setiap dokter akan senang untuk berbagi pengetahuan demi kesembuhan pasiennya. Jadi jelas sudah bahwa komunikasi antara pasien-dokter merupakan salah satu unsur penting yang dapat membantu seorang pasien sembuh dari penyakitnya. (Andi W, dr) *</p>

Setiap orang bisa melepaskan diri dari candu nikotin, yang meski membuat kondisi ketagihan namun tidak sekuat efek ketagihan yang diciptakan oleh narkotika dan obat-obatan terlarang (narkoba). Kunci dari keberhasilan melepaskan diri dari kecanduan adalah mampu melakukan pengendalian diri. Bukankah itu juga yang menjadi salah satu tujuan berpuasa, mengendalikan diri dari hawa dan nafsu?</p>
<p>Banyak para perokok yang faktanya mampu menahan keinginan untuk merokok, setidaknya untuk rentang waktu dari Subuh hingga Magrhib. Jika mampu menahan hingga selama itu, mengapa tidak dilanjutkan hingga ke waktu tarawih lalu bertemu subuh lagi? Menghilangkan kebiasaan memang bukan sesuatu yang mudah, apalagi melepaskan diri dari kecanduan nikotin.</p>
<p>Rentang waktu satu bulan selama Ramadhan merupakan media yang tepat untuk latihan menghentikan kebiasaan merokok. Latihan bisa diawali dengan mengurangi kebiasaan merokok setelah makan. Ganti rokok dengan cemilan lain, permen misalnya, atau mungkin gosok gigi. Jika berhasil menahan rokok usai makan, itu adalah langkah awal yang sangat baik.</p>
<p>Selanjutnya secara bertahap upayakan terus memanjangkan rentang waktu tidak merokok. Mulai sampai berbuka, lalu sampai tarawih, lalu sampai sahur, dan terus tidak merokok sepanjang Ramadhan. Jika berhasil, meski tidak sepenuhnya berhenti merokok total, jangan pernah melepaskan keberhasilan itu. Lanjutkan terus perang terhadap keinginan merokok di hari-hari setelah Ramadhan usai.</p>
<p>Yang pasti, sekuat apapun keinginan kita untuk menghentikan merokok, tidak akan berhasil tanpa benar-benar bertekad dan melaksanakan upaya-upaya yang nyata. Jadi, pertanyaan yang harus dijawab adalah, mampukah Anda mengendalikan diri sendiri? atau hidup Anda memang selalu dikendalikan oleh pihak lain termasuk dikendalikan nikotin?</p>

<p><a href="http://sehati.net/wp-content/uploads/2009/03/pregnant.jpg"><img class="alignnone size-medium wp-image-38" title="pregnant" src="http://sehati.net/wp-content/uploads/2009/03/pregnant-300x199.jpg" alt="" width="300" height="199" /></a>Folat alias asam folat atau folasin, memang disarankan untuk dikonsumsi dalam jumlah yang cukup setiap hari oleh semua wanita pada usia subur, terutama untuk wanita yang berencana ingin segera hamil atau dalam kondisi kesehatan tertentu yang menyebabkan kadar folat dalam tubuhnya rendah. Folat sangat diperlukan terutama pada tahap-tahap awal kehamilan, sebagai upaya pencegahan cacat janin. Folat dipercaya berperan penting dalam upaya pencegahan kejadian cacat janin bawaan, terutama yang berhubungan dengan cacat pada selubung (tabung) saraf dikenal dengan istilah Neural Tube Defect (NTD).</p>
<p>NTD biasanya terjadi karena kekurangan folat pada proses pembentukan organ-organ janin (organogenesis), yaitu pada usia kehamilan kurang lebih dua hingga empat minggu (dihitung sejak terjadinya pembuahan). Sayangnya, seorang wanita baru menyadari dirinya hamil setelah proses penting tersebut terlewati, yaitu saat usia kehamilannya sudah mencapai lima hingga enam minggu. Agar tidak sampai  “kecolongan” maka sangat disarankan setiap wanita memperhatikan kecukupan jumlah folat bahkan sebelum dirinya hamil.</p>
<p>Ada tiga tipe kecacatan NTD, antara lain Sfina Bifida, Ensefalokel dan Anesefal. Bentuk tersering adalah Sfina Bifida (sekira 65% dari seluruh kasus NTD). Ada dua jenis Sfina Bifida, ada yang tampak dan tidak tampak, pada prinsipnya terdapat lubang menganga di tulang belakang karena ada ruas tulang belakang yang gagal bertaut. Bentuk lainnya adalah ensefalokel dan Anesefal. Bayi yang terlahir dengan kondisi anesefal tidak akan bertahan lama, karena mereka biasanya terlahir tanpa tempurung kepala, otaknya hanya dilapisi oleh selubung tipis dan transparan.</p>
<p>Berdasarkan penelitian diketahui bahwa pemberian folat akan menurunkan risiko kecacatan sampai sekitar 50-85%. Hal ini dikarenakan fungsi penting folat sebagai koenzim dalam proses sintesis asam nukleat, yang dibutuhkan dalam pembelahan sel. Karena pembelahan sel sangat cepat pada tahap organogenesis, maka kekurangan folat dapat menjadi faktor risiko terjadinya cacat bawaan. Faktor risiko terjadinya NTD pada sebuah kehamilan ditentukan oleh faktor genetik, lingkungan, riwayat keluarga yang mengalami hal serupa, kekurangan gizi serta wanita yang menghadapi risiko kesehatan tertentu. Misalnya, wanita dengan kencing manis yang perlu pemberian insulin, wanita yang meminum obat antiepilepsi (valporic acid, carbamazapine), wanita yang meminum obat-obat antagonis asam folat (amniopterin, methotrexate).</p>
<p>Tentunya, untuk wanita yang memiliki faktor risiko tersebut di atas, pemberian folat perlu diberikan dengan jumlah yang cukup setiap hari, tanpa memperhatikan apakah ia hamil atau tidak. Sebaiknya setiap wanita yang merencanakan kehamilan menceritakan kondisi kesehatannya pada dokter sehingga bisa diberikan suplemen folat sesuai dengan kebutuhan.</p>
<p>Folat termasuk kelompok vitamin B, yaitu vitamin B9. Vitamin yang larut dalam air ini, banyak terdapat pada sayur-sayuran berwarna hijau, misalnya bayam dan brokoli. Juga terdapat dalam buah-buahan seperti jeruk, semangka, nanas dan alpukat. Menurut Prof.dr Andrew E Czeizel  dari Faculty of Genetics, University of Sciences-Honggaria, ada tiga cara untuk mendapatkan kecukupan folat, yaitu makan banyak sayuran, buah, biji-bijian, mengkonsumsi suplemen folat atau makan-makanan yang difortifikasi. Jika seorang wanita memperhatikan pola makan dengan cukup sayuran dan buah-buahan setiap harinya, maka kadar Folat dalam sumber alamiah tersebut sebenarnya sudah mencukupi. Sayangnya dalam pola makan “modern”  dan cenderung bersifat instan seperti saat ini, menu sayuran dan buah-buahan kadang terlupakan.</p>
<p>Mengingat besarnya manfaat folat untuk wanita, maka sebaiknya wanita selalu berusaha memenuhi kebutuhan folat harian dalam jumlah yang cukup, jadi jelas bukan hamil gak hamil, folat tetap penting. *</p>
